Friends, Family Members, and Disaster Organizations:
With your agreement, we may share your PHI with friends, family, or disaster organizations to assist with treatment or payment.In emergencies, we may share PHI as necessary unless you have previously instructed us otherwise.
Authorizations:
Uses and disclosures not described above require your written authorization.We do not sell your PHI or use it for marketing without your written permission.You can revoke your authorization at any time by contacting our Privacy Officer.
State Law and Disease Specific Privacy Protections:
We comply with state laws that require written consent before using or disclosing information related to certain conditions, such as sexually transmitted infections.
Your Rights Regarding Your PHI

Request Restrictions:
You may request restrictions on how we use or disclose your PHI. We are not required to agree unless it involves a service paid out-of-pocket and the disclosure is to your insurer.
Notification of Breach:
You will be informed promptly if a breach of your unsecured PHI occurs.
Access to PHI:
You can request paper or electronic copies of your PHI used to make decisions about you. A reasonable, cost-based fee may apply.
Request Changes:
You can request corrections to your PHI. We will review and make changes if reasonable. If we deny the request, we will provide a written explanation within 60 days.
Preferred Communication:
You can ask us to communicate with you in a specific way or send mail to a different address. We will accommodate reasonable requests.
Accounting of Disclosures:
You can request a list of who accessed or received your PHI and for what purpose, covering the previous six years. Routine disclosures for treatment, payment, and operations are not included. One free accounting per year; additional requests may incur a fee.
Complaints:
You can complain about our privacy practices to us or to federal or state agencies. We will not retaliate for filing a complaint.

You can request a copy of this notice at any time. Changes to this notice apply to all information we hold. The latest version is posted on our website.
